It is feared by psurlons, tsochari, and nilshai, but few other beings know of it, including a small number of alienists, cultists, and half-farspawn. Its goals are unknown, but it is hostile to all life. Other similarly powerful and malevolent Far Realm entities also exist, each with their own followers. It was described as an enormous thrashing worm-like thing.

One of Mak Thuum Ngatha nine tentacles is beneath Hanging Glacier and is worshiped as the "Frost Worm" by a group of cultists led by Tarja Arjotta (Male, Human (Suel) Cleric).[1]

Another tentacle is in a pit beneath Oblestone Keep in Onnwal, where it is kept at bay by wards from the Aerie.[2]
