Greyhawk Tournament History (including very early OD&D tourneys)

I am attempting to collect a definitive listing of all known Greyhawk tournament module adventures, especially those written by published/established Greyhawk authors.  Links below are primarily to The Acaeum, Adrian Newman's TSR Archive, or to The Tome of Treasures, although in a few cases I have (or will have) documents on my site about the tournament in question. Randy Porter, the Keeper of Ancient GenCon Lore, is another very useful resource.

I have trolled through the Dragon Magazine Archive, and tried to build a listing of likely candicates from the Convention Calendar (or its equivalent in The Strategic Review and The Dragon).  According to Erik Mona, the RPGA database of scenarios was purged around 1987, so WotC/TSR doesn't have copies of any scenarios prior to that timeframe (other than those in the hands of players, judges, and authors).

unknown 1984 tourney R6 Bigby's Tomb was used at an unknown tournament in 1984. It was later published in Polyhedron #20 (and retitled The 384th Incarnation of Bigby's Tomb). Incidentally, code R6 was originally assigned to a planned module by Eric Shook, "The Eye of the Bog". This module was used at GenCon South in 1981 or 1982, but never published outside the tournament -- nor were any tournament copies given to players (thanks to Eric Shook for help with this info). Eric Shook founded Doorway Games to publish this adventure as well as Axe of the Executioner Eric Shook (see also Dragon 65, p. 37)
